The information and the documents provided are true, accurate and complete, and I will notify Jadenex without delay of any material change before a decision is made. I understand that a listing decision is at the absolute discretion of the Jadenex listing committee, that no fee is accepted in exchange for a decision, and that an asset may be suspended or removed under the listing policy. I consent to Jadenex screening the entity, its directors and its beneficial owners against sanctions, politically exposed person and adverse media sources, and to the retention of this application in accordance with the privacy policy.

A reference is issued on submission. Assessment takes fifteen business days from the date a complete application is received.
